Patient Participation Group - Primary Care Networks Focus Group

We'll be gathering thoughts on how Primary Care Networks will fit into the NHS Long Term Plan. 

Primary Care Networks are GP surgeries working more closely with other services such as hospitals and pharmacies to improve care for people with long term conditions.
